CHILD & FAMILY TEAMS

MonmouthCares is not a treatment or social service program. We help families find, create and develop options that will support them while they work on solutions to the challenges they face. The Child and Family Team is a crucial part of this process. The Team helps families help themselves and gives them the support they need to go forward.

MonmouthCares helps Teams by providing the structure for the Team meetings and by helping put in place the Individual Service Plan that the Team creates. This is called “Care Management.”

Care Management is a voluntary process. If a caregiver refuses to or cannot be a member of a Child and Family Team, Care Management cannot be provided. Children and families that cannot use a Team can still receive services through other systems and organizations within the New Jersey Division of Child Behavioral Health System of Care for Children.

what constitutes a Child and Family Team?

The Child and Family Team is made up of the people who know the child and family best. Team members are people who are willing to make a commitment to do whatever it takes to help the child and family make their life better.
